The FIM System® is a an outcomes management program for skilled nursing facilities, subacute facilities, long-term care hospitals, Veterans Administration programs, international rehabilitation hospitals, and other related venues of care. The FIM System® enables providers and programs to document the severity of patient disability and the results of medical rehabilitation and establishes a common measure for the comparison of rehabilitation outcomes.

The FIM System® provides an established means of collecting rehabilitation data in a consistent manner. It allows clinicians to follow changes in the functional status of their patients from the start of rehabilitative care through discharge and follow-up.

At the core of The FIM System® is the FIM™ instrument, a valid and proven rating tool that can be administered quickly and uniformly. Long regarded as the gold standard for measuring function, the FIM™ instrument is widely recognized by, and familiar to, most rehabilitative staff. This familiarity, coupled with minimal administrative burden, makes implementation easy, and subscribers can produce meaningful reports soon after.

By combining FIM™ assessments with clinical, financial, and demographic data, subscribers can

  • evaluate the efficiency and effectiveness of their rehabilitation programs;
  • manage team conferences and enhance case management with the help of FIM™ profile reports;
  • support communication with stakeholders, including families, payers, referral sources, and governance; and
  • generate data for the accreditation processes of The Joint Commission, CARF, and other regulatory agencies.

SNF/Subacute

  • After receiving a signed agreement from your facility, UDSMR will send an implementation kit to your facility.
  • Once you have successfully accessed your UDS-FIMi™ account, you can explore the many features of the system and begin data entry.
  • Review clinical materials to prepare clinicians for implementation and the credentialing process.
  • Assessments are performed upon admission and discharge. Interim and follow-up assessments are optional.
  • Immediately following data entry, you can create numerous real-time data reports that help you facilitate care planning, team meetings, and management decisions.
  • Each quarter, you will receive national benchmark reports that provide comprehensive data for evaluating your program.

LTCH

UDSMR has long recognized the importance of long-term care hospitals (LTCHs) in the post-acute care continuum. LTCHs have used the FIM™ instrument to monitor the functional improvement of some of the most impaired patients for more than 14 years. UDSMR’s LTCH subscribers are a mix of medical and rehab LTCH models. UDSMR’s national database for long-term care hospitals is specific to hospitals with average lengths of stay of 25 days or more.
